You just need to watch your opponent and draw when you have enough 'dead eye' time to hit the target you want. Apart from the actions of your opponent, you also need to consider what you want to aim at once you have drawn.
If you don't want to shoot to kill you may require a longer time to aim. If you have waited too long to draw and you don't have time to aim before the opponent shots you, then you can press R2 rapidly again after you have drawn. This usually results in a body shot to your opponent, provided they haven't moved out the way, and will usually kill them. I have an elite xbox controller and was experiencing the same thing for one of the story missions.
It dodnt matter how fast I pressed the button, there was no opportunity to also press it completely. To get around this, I had to map an extra button also to RT. So i used RT to slow press, then pressed my newly mapped "RT" to draw. Old controllers for some reason can not detect the second the command after you hold the right trigger.
I switched out the button assignments to see if I had to tap the right trigger in order to for my draw my handgun and I was right. My controller that I bought 1 year ago works fine with duels but my black controller that came in with my PS4 in is not working too well with duels. The next phase is all about aiming at your opponent. This is where you get the chance to find the spot where you want that bullet to go. Obviously, aiming for the head or the heart means certain death, but providing you have your wits about you, aiming for the hand will knock the gun out of their grip, disarming your opponent. And the one bit of advice I remember the game giving me, to "not draw too quickly, because my shots would be inaccurate" or something, doesn't seem to make any sense, since I can manually control where the shots go. I had trouble with dueling too. Perhaps try shooting where your crosshair lights up red. Also wait until dead-eye automatically engages and it tells you to draw. You can also try disarming the guy by shooting him in the hand.
Some people, usually story related, cannot be disarmed, as such aiming for the hand will only get you killed instead, which can be frustrating.
Every trigger pull increases your duel meter, which needs to be higher than the other guy's to win. You get more duel meter off some parts of the body than others -- like head or gun hand. You also get more from a white crosshair than red. Shooting the opponent's gun hand will often instantly end the fight. The opponent will remain alive, be disarmed, and you'll get significantly more Fame points.
You have to do a couple of things to correctly duel. First, when it says draw, you need to pull down on the stick until John grabs his gun. Then, line up the crosshairs until they turn white, the enemies gun hand or head arethe easiest, and fire. I recommend NOT using a pistol but a revolver for a duel. Most duels you have to go to the starting point to initiate the duel. Switch to a revolver. In the little that I dueled with the semi-auto or the mauser, since they can hold quite a few bullets, they take a bit longer to fire.
